The online health assessment does not use cookies, analytics, advertising trackers, or session recording of any kind, whatever you choose above. Nothing you enter there is shared with an advertising platform.

We do not share, sell, or provide your mobile phone number or messaging consent data to third parties or affiliates for marketing or promotional purposes. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes, and text messaging opt-in data and consent are excluded from every category of sharing described in our Privacy Policy.

Submitting a privacy request

To request access to, correction of, or deletion of your personal information, or to opt out of the sale or sharing of personal information, email legal@joinsoha.com.

Health information held by the medical group that provides your care is governed by HIPAA and by its Notice of Privacy Practices, which explains how to exercise your rights in that information.
